Spoiler-alert! If you have seen this zombie film: Why do you think the movie is called the way it is? Think about it, before reading on. My guess is, you won't stand a chance to come up with the reason.
Originally, scientist Robert (Will Smith) does not heal his female zombie-like patient in his house, but surrenders her to her zombie-lover and the other zombies, who let Robert go. He realizes that they don't want to get healed, and that these creatures are Earth' new Human society now. Robert and the other remaining uninfected few Humans are the real disruptive force now. And they all get along happily thereafter, presumedly.

4: The reality behind this: Every word is true. You can watch the alternate ending here: https://www.youtube.com/watch?v=kPSk30qzgFs βοΈ The book has also been adapted with Charlton Heston as "The Omega Man" in 1971 and as a low budget version "I Am Ξ©mega", in 2007 as well. However, the story adaptation with Vincent Price is the most intense of all.
3: Of course, the black & white old movie has aged. But: Because it's story is so captivating, and it was filmed in a real European futuristic looking architecture scenery back then, it has become a classic, an insiders' tip. Whereas the slick eye candy Hollywood production's CGI of 2007 is aging rapidly. With no gripping storyline, espcially in the theatrical cut, to stop its free fall. Hollywood these days doesn't even care wether their movie titles make any sense whatsoever anymore, once they have butchered the classics' stories.
2: Only to find out in the end that there are two versions of vampires having emerged. Brutal mindless monsters and an intelligent new society. The latter is killing the monsters, too, during the night, cleaning up Earth. Plus having to terminate the most dangerous monster of all, the last Human remaining, the scientist, who kills off the intelligent ones indiscriminately during the day. Thus, Vincent Price realizes, hence too late, that HE is the real monster on Earth, everyone else remaining intelligent on the planet is afraid of. With nobody wanting to get "healed", i.e. transformed. Thus, the last Human has become the legendary monster - after he gets "cleaned up" by the new society.
1: The producers, i.e. the film studio pressured the filmmaker to end the movie differently, presumedly more palatable to bland mainstream taste. However, the original ending is much closer to the book the filmmakers had adapted. But not even as grim as the first US-Italien adaptation with Vincent Price of 1964, "The Last Man on Earth". Back then, humanity turned into vampire-like creatures, the scientist is killing during the day.
